₹5 lakh cashless health insurance per family per year for ALL J&K domicile holders — unique in being universal (not limited to BPL); covers hospitalisations, surgeries, cancer, cardiac care, dialysis, orthopaedic procedures and 1,500+ medical packages at empanelled government and private hospitals across India; includes ₹1,500 transport allowance per hospitalisation; also covers pre-existing diseases from day 1; launched by PM Modi in 2020 as extension of Ayushman Bharat (cover up to ₹5,00,000)
Aadhaar Card (mandatory — scheme is Aadhaar-seeded), Domicile Certificate (J&K), SEHAT / Ayushman Card (generated on portal), Family composition details (Aadhaar of all members).
