Persons with disabilities of any age living in Uttar Pradesh. Family income within the poverty line (₹46,080 a year in villages, ₹56,460 in towns). Device recommended by a medical officer.
Up to ₹8,000 per person to buy or repair aids — tricycle, wheelchair, CP chair, calipers, crutches, hearing aid, education kit — as recommended by a medical officer. (about ₹8,000 one-time)
Disability Certificate, Income Certificate, Aadhaar Card, Medical officer's prescription for the device, Photo.
